Professional Management of Outbreak Investigations in Northern Nigerian States: Capacity Assessment and Recommendations

Epidemic outbreak investigation capacity in northern Nigerian states determines the speed and effectiveness of containment responses to cholera, meningitis, and typhoid outbreaks that recur in the region. This study assesses professional outbreak investigation capacity in health departments of Borno, Kebbi, and Gombe States, North East and North West Nigeria, focusing on laboratory confirmation speed, epidemiological investigation competency, and coordination between state and federal levels. A mixed-methods design combines structured capacity assessments at six state reference laboratories, interviews with 30 public health officers and laboratory scientists, and review of outbreak investigation reports from 2020 to 2024. Significant deficits were identified in culture confirmation infrastructure, with two of six laboratories lacking functional anaerobic culture capability. Average time from outbreak alert to laboratory confirmation exceeded 7 days in all states. Epi-curve construction and source attribution analysis were absent from 65 percent of reviewed outbreak reports. Coordination delays between state epidemiology units and NCDC were documented in four outbreak episodes. The study proposes a Northern Nigeria Outbreak Response Capacity Framework with tiered competency standards, minimum laboratory equipment requirements, and inter-state mutual aid agreements. Professional recommendations include NCDC-funded laboratory equipment grants and mandatory Field Epidemiology Training Programme enrolment for state outbreak investigators.
