Trabecular Meshwork Biomarkers and Aqueous Humour Proteomics in Nigerian Glaucoma Patients: An Original Contribution to Disease Mechanism Research

The trabecular meshwork (TM) regulates aqueous humour outflow, and its dysfunction is central to the pathogenesis of primary open-angle glaucoma. Aqueous humour proteomics offers a window into disease-specific molecular alterations in the glaucomatous TM, yet no such study has been conducted in West African populations. This study investigates TM biomarkers and aqueous humour proteomic profiles in Nigerian glaucoma patients and controls, using samples collected perioperatively at the Lagos University Teaching Hospital and Aminu Kano Teaching Hospital. A case-control biomarker study design will be employed, collecting aqueous humour samples from 60 glaucoma patients undergoing trabeculectomy and 60 non-glaucomatous controls undergoing cataract surgery. High-resolution mass spectrometry-based proteomics will identify differentially expressed proteins. Candidate biomarkers will be validated using enzyme-linked immunosorbent assay (ELISA). Bioinformatics pathway analysis will characterize molecular pathways implicated in the Nigerian glaucoma proteome. The study will produce the first aqueous proteomics dataset for Nigerian glaucoma patients, constituting a fundamental original scientific contribution to glaucoma molecular biology in Africa. Findings will be published in peer-reviewed international journals and submitted to the WHO's glaucoma research consortium. This research advances mechanistic understanding of glaucoma in the African genetic context and provides a foundation for biomarker-based early diagnosis.
