Relationship Between Systemic Hypertension and Hypertensive Retinopathy in Adults in Enugu State: An Analytical Study

Hypertensive retinopathy reflects end-organ vascular damage caused by chronically elevated blood pressure and serves as both a clinical indicator of systemic hypertension severity and an independent predictor of cardiovascular risk. This study analyses the relationship between systemic hypertension severity and hypertensive retinopathy grading among hypertensive adults in Enugu State, South East Nigeria. An analytical cross-sectional study will be conducted at the medical outpatient clinic of UNTH Enugu among 300 adults with confirmed systemic hypertension. Blood pressure will be measured using a standardised protocol. Fundus examination will be performed by a senior ophthalmology resident using the indirect ophthalmoscope and categorised using the Keith-Wagener-Barker grading system. Duration of hypertension, antihypertensive medication type and adherence, HbA1c, serum creatinine, and body mass index will be documented as covariates. Ordinal logistic regression will analyse the relationship between hypertension severity and retinopathy grade. Despite the high prevalence of hypertension in South East Nigeria, hypertensive retinopathy is frequently missed because routine fundoscopy is not standard practice in medical outpatient clinics. This contributes to late detection of potentially reversible end-organ damage. This study provides Enugu State-specific data and makes a clinical case for integrating ophthalmology screening into hypertension management clinics. Recommendations will address training of internal medicine residents in fundoscopy, inter-departmental referral protocols, and patient education on the ocular consequences of uncontrolled blood pressure.
