Developing and Testing a Community-Based Early Warning and Referral System for Maxillofacial Emergencies in Rural Communities of North Central Nigeria

Maxillofacial emergencies including deep space infections, Ludwig's angina, facial fractures, and traumatic hemorrhage carry high preventable mortality in rural Nigeria due to delayed recognition, poor first aid, and absence of structured emergency referral pathways. No community-based early warning system specifically targeting maxillofacial emergencies exists for rural settings in the North Central geopolitical zone. This intervention PhD study designs, implements, and evaluates a community-based early warning and referral system (CBEWRS) for maxillofacial emergencies in 12 rural communities across Benue, Kogi, and Nasarawa States, with intervention delivery through trained community health volunteers (CHVs) and mobile technology-enabled referral coordination. Using a participatory action research design, the CBEWRS was co-designed with community representatives, CHVs, and district hospital clinicians through iterative co-design cycles. The intervention included CHV training in maxillofacial emergency recognition, standardized referral trigger criteria, mobile phone-based digital referral forms, and coordinated ambulance dispatch agreements with two district hospitals. Implementation was evaluated over 18 months using a mixed-methods approach combining quantitative outcome data (referral rate, emergency presentation time-to-hospital, case severity at arrival, morbidity) and qualitative process evaluation data. Results indicate a 44% reduction in median time from symptom recognition to hospital arrival in intervention communities compared to matched control communities. Referral appropriateness increased from 38% to 74% post-CBEWRS implementation. The study provides proof-of-concept for a scalable community-based maxillofacial emergency system adaptable to other geopolitical zones.
