Assessment of Telemedicine Implementation and Patient Satisfaction in Rural Health Outreach Programmes in Osun State, South West Nigeria

Telemedicine offers a transformative opportunity to extend specialist medical consultation to underserved rural populations in Nigeria, but implementation quality and patient acceptance in South West state outreach programmes have not been rigorously assessed. This study assesses the implementation fidelity, operational challenges, and patient satisfaction of telemedicine services within the rural health outreach programme of the Osun State Ministry of Health. A mixed-methods cross-sectional evaluation design is applied across six rural telemedicine outreach sites in three local government areas. Implementation assessment uses the WHO Framework for Digital Health Interventions to evaluate connectivity reliability, device functionality, specialist consultation availability, documentation standards, and referral follow-through. Patient satisfaction is measured among 240 telemedicine service users using a validated adapted SERVQUAL instrument. Qualitative data from focus group discussions with programme officers are analysed thematically. Available telemedicine evaluation literature from South West Nigeria identifies unstable internet connectivity and inconsistent specialist availability as the most critical implementation constraints, with rural patients generally expressing high satisfaction when services function as intended. Findings will provide the Osun State Ministry of Health with actionable recommendations on infrastructure investment, specialist scheduling, and community mobilisation for telemedicine programme strengthening.
