Analysis of Referral Patterns and Communication Quality Between Primary and Tertiary Health Facilities in Kaduna State, North West Nigeria

Effective referral communication is a critical component of safe patient transfers between primary and tertiary health facilities, yet the quality of referral documentation and inter-facility communication in North West Nigeria is poorly evaluated. This study analyses the completeness of referral documentation, timeliness, and communication quality of patients transferred from primary health centres to the Kaduna State Government Hospital tertiary system. A retrospective review design is applied to referral letters and corresponding emergency department case notes of 280 referred patients received over a 12-month period from 2022. Variables assessed in referral documents include legibility, primary diagnosis clarity, vital signs and investigation results provided, treatment administered before transfer, and contact details of referring clinician. Concordance between referring diagnosis and receiving diagnosis is evaluated. Structured interviews with 30 receiving emergency physicians assess communication quality perceptions. SPSS is used for quantitative analysis, and interview data are analysed thematically. Available health systems referral literature from North West Nigeria identifies missing vital signs data and absent contact information in over 55 percent of referral letters as critical quality gaps. The SBAR (Situation, Background, Assessment, Recommendation) communication framework provides the quality assessment reference. Findings will support Kaduna State's health authorities on standardising a structured referral form and implementing SBAR training at primary health centres.
