📖 ABSTRACT/OVERVIEW
Health information systems are vital for data-driven healthcare management, but their implementation in conflict-affected and resource-constrained primary healthcare environments such as Borno State faces unique challenges. This study assesses the implementation status and operational challenges of health information systems at primary healthcare centres in Borno State, North East Nigeria. A descriptive survey was conducted across twenty-two PHCs in Maiduguri and Jere local government areas, engaging 165 health workers and ICT officers through structured questionnaires and direct observation checklists. Variables assessed included hardware availability, software deployment status, staff training levels, data entry frequency, system uptime, and data quality. Descriptive statistics and frequency analysis were applied in SPSS. Available health informatics literature from North East Nigeria identifies intermittent power supply, damaged hardware, and high staff turnover following displacement as the dominant barriers to sustained HIS functionality in Borno PHCs. The Delone and McLean IS Success Model provides the evaluative framework. Findings indicate that only 38 percent of assessed PHCs had fully operational electronic health information systems, with paper-based data collection remaining dominant in rural settings. The study recommends solar-powered computing stations, robust training programmes that survive staff rotation, and simplified mobile data collection tools compatible with DHIS2 as priority investments for Borno State Ministry of Health.
