Patient Dose Assessment in Fluoroscopy-Guided Interventional Procedures at University College Hospital, Ibadan

📖 ABSTRACT/OVERVIEW

Fluoroscopy-guided interventional procedures deliver substantially higher radiation doses than conventional radiography, and dose assessment in this procedural category is essential for patient safety, operator protection, and regulatory compliance in Nigerian tertiary centres. At University College Hospital Ibadan, a range of fluoroscopy-guided procedures are performed weekly, yet systematic radiation dose auditing in this procedural category remains limited. This study assesses patient radiation doses during fluoroscopy-guided interventional procedures at University College Hospital, Ibadan. A prospective observational design is employed over a three-month data collection period, recording air kerma area product values, fluoroscopy time, and number of digital subtraction acquisition runs for 120 consecutive fluoroscopy-guided procedures across barium studies, hysterosalpingography, and vascular interventions. Patient demographic and procedural complexity data are concurrently recorded. Descriptive statistics and one-way ANOVA comparing dose values across procedure categories are computed in SPSS. Available fluoroscopy dose literature from South West Nigerian hospitals identifies prolonged fluoroscopy times and absence of dose area product meters as the primary dose monitoring limitations in many teaching hospital interventional suites. The IAEA Dosimetry in Diagnostic Radiology publication and the European Commission diagnostic reference levels for interventional radiology provide the normative benchmarks. Findings are expected to reveal dose-area product values exceeding reference levels for complex vascular procedures. Recommendations address fluoroscopy time management training, pulse fluoroscopy adoption, and dose area product monitoring as a standard procedural record at UCH Ibadan.
