Original Development of a National Quality Assurance Framework for Diagnostic Ultrasound Services in Nigeria: Standards, Implementation, and Governance

Nigeria lacks a national quality assurance framework for diagnostic ultrasound services despite ultrasound being the most widely deployed diagnostic imaging modality in the country, with implications for service quality consistency, equipment safety, and patient care outcomes across thousands of public and private imaging facilities. This study develops and validates an original National Quality Assurance Framework for Diagnostic Ultrasound Services in Nigeria. A framework development methodology comprising five phases is applied: stakeholder needs assessment through a national survey of 400 ultrasound practitioners and 50 health administrators identifying current QA deficiencies and priority domains; systematic review and synthesis of national ultrasound QA frameworks from the United Kingdom, United States, Australia, and comparable African healthcare systems; co-design of the framework through participatory workshops with 60 multi-stakeholder representatives including NRBN, NNRA, MDCAN, hospital managers, and patient advocates over six months; framework pilot implementation and feasibility testing across 25 hospitals in six geopolitical zones over twelve months; and validation through Delphi consensus with 35 national and international ultrasound quality experts. The original National Ultrasound QA Framework encompasses equipment performance standards, practitioner competency requirements, clinical protocol standards, governance structures, and implementation roadmap. A novel Nigerian Ultrasound QA Maturity Index is developed for progressive implementation across different healthcare system levels. Available ultrasound QA literature from Nigeria identifies the complete absence of a national framework as the fundamental quality governance gap. The WHO Quality Management Framework and AIUM practice guidelines provide the international reference.
