Application of the Patient-Centred Medical Home Model in a Nigerian General Practice Setting in FCT Abuja

Background: The Patient-Centred Medical Home model represents a comprehensive approach to coordinating primary care that has been increasingly applied in developing country settings. This study examines the feasibility and preliminary outcomes of applying the Patient-Centred Medical Home model in a general practice clinic in the Federal Capital Territory, Abuja. Methods: A programme evaluation design assessed the model's implementation fidelity, patient engagement, and health outcome metrics over a 12-month period. Data were collected through structured patient interviews, clinical record reviews, and provider surveys from a sample of 180 enrolled patients. Results: Patients enrolled in the PCMH model demonstrated improved appointment adherence and blood pressure control compared to pre-implementation records. Integrated care coordination reduced emergency department visits. Provider satisfaction was generally high. Barriers included insufficient staffing ratios and inadequate electronic health record systems. Conclusion: The Patient-Centred Medical Home model shows promise for improving care quality and patient outcomes in Nigerian general practice contexts. Structured training of family medicine practitioners and investment in health information technology are prerequisites for successful national scale-up.
