Assessment of Patient Record Management Practices and Data Quality at Health Centres in Jigawa State

Background: Accurate patient records are foundational to quality healthcare delivery, continuity of care, and health system planning. This study assesses patient record management practices and data quality at selected primary health centres in Jigawa State, North West geopolitical zone. Methods: A facility-based assessment was conducted at 15 primary health centres using validated data quality assessment checklists. Key informant interviews were conducted with records officers and facility managers. Quantitative and qualitative data were triangulated in the analysis. Results: Paper-based record systems were universal across assessed facilities. Data completeness rates were low, with vital status updates and follow-up documentation being the most frequently missing elements. Storage conditions were inadequate in most facilities, with records susceptible to moisture and pest damage. Staff training in records management was minimal. Conclusion: Patient record management at primary health centres in Jigawa State requires urgent systemic reform including investment in digital health infrastructure, staff capacity building, and policy frameworks mandating data quality standards. Family medicine practitioners should be involved in health information system strengthening at the facility level.
