Background: Effective referral from primary to secondary and tertiary care is a cornerstone of a functional health system, yet the referral pathway in Nigeria is frequently bypassed or dysfunctional. This study explores the barriers to specialist referral from primary care facilities in Imo State, South East geopolitical zone, from the perspective of primary care providers. Methods: A qualitative study using semi-structured in-depth interviews was conducted with 24 primary care physicians and nurses across 12 facilities in Owerri and surrounding LGAs. Thematic analysis was conducted using NVivo version 12. Results: Key barriers identified included patient financial constraints, absence of a structured referral tracking system, poor communication between referring and receiving facilities, and inadequate capacity for clinical decision-making regarding appropriate referral criteria. Patients' distrust of tertiary facilities was also highlighted. Conclusion: Referral barriers in Imo State undermine continuity of care and patient safety. Family medicine practitioners should develop structured referral protocols with feedback mechanisms and engage policymakers to establish formal inter-facility communication channels supported by digital health platforms.
