Birth registration rates in local government areas of Oyo State remain below 50 percent in many communities, partly because existing manual registration processes are slow and geographically inaccessible to rural populations. This study presents the development of a digital birth registration system designed to improve registration efficiency and coverage in a local government area of Oyo State, South West Nigeria. The system was built using PHP, MySQL, and a Bootstrap-based interface, and includes a mobile component enabling community health workers to initiate registration in the field with subsequent synchronisation to the central server. Core features include parent and child data capture, document upload (photographs, hospital delivery notes), certificate generation, and an administrative dashboard for LGA civil registry staff. Requirements were gathered from civil registry staff, community health workers, and four community leaders. Development used Rapid Application Development with three iterative cycles. Functional testing validated data validation rules and certificate generation across 80 test scenarios. A four-month pilot in Afijio LGA registered 540 births, representing a 31 percent increase over the same period in the prior year. Community health workers reported a 65 percent reduction in the time required to complete a registration compared to the previous paper process. The study recommends partnership with UNICEF and the National Population Commission to scale the system across all LGAs in Oyo State.
