Forensic Documentation of Industrial Workplace Accidents in Oil and Gas Companies in Akwa Ibom State

Industrial workplace accidents in the oil and gas sector require rigorous forensic documentation to support regulatory compliance, civil litigation, and safety improvement processes. This study examines the forensic documentation practices applied in industrial accident investigations at oil and gas facilities in Akwa Ibom State between 2020 and 2024. A professional practice assessment design was employed, collecting data from interviews with eighteen health, safety, and environment managers, twelve regulatory officers from the Department of Petroleum Resources, and review of twenty-five accident investigation reports. The study evaluates the completeness of physical evidence documentation, photographic standards, witness statement protocols, and the integration of forensic findings into regulatory enforcement actions. Findings indicate that most internal accident investigation reports meet minimum regulatory documentation requirements, but fall short of the evidentiary standards required for civil litigation, particularly regarding chain of custody and independent verification of physical evidence. Photographic documentation was universally employed, though systematic measurement and scene diagramming were present in only fifty percent of reports. Expert independent forensic involvement was rare, occurring in fewer than twenty percent of serious injury cases. Recommendations include the development of a sector-specific forensic accident investigation protocol for Nigeria's oil and gas industry, mandatory independent forensic review for fatality cases, and enhanced capacity building for HSE professionals in evidence collection and reporting.
