Implementation of Patient Medication Counseling Programs in Government Hospitals: A Performance Review in Ogun State, South West Nigeria

Structured patient medication counseling programs are evidence-based interventions that improve adherence, reduce medication errors, and enhance patient satisfaction. Despite their demonstrated effectiveness, their systematic implementation in Nigerian government hospitals remains inconsistent. This study reviewed the performance of patient medication counseling programs in government hospitals in Ogun State, South West Nigeria. A cross-sectional mixed-methods study was conducted across eight government hospitals, combining patient exit interviews with 240 patients, pharmacist structured interviews with 35 pharmacy staff, and review of pharmacy operational protocols. Counseling program performance was assessed against benchmarks adapted from the American Society of Health-System Pharmacists counseling standards. Results indicated that only four hospitals had formally documented counseling protocols. Counseling was provided during fewer than 45 percent of dispensing encounters, and duration averaged less than 90 seconds per interaction. Content adequacy, assessed against a six-point checklist, was satisfactory in only 32 percent of observed encounters. New medication counseling was rated better than refill counseling. Patient satisfaction with counseling was low at 2.8 out of 5. Pharmacist-to-patient ratios exceeding 1:120 per shift were identified as the most critical operational barrier. The study recommends adoption of standardized counseling templates, appointment of dedicated counseling pharmacists, and establishment of counseling performance key performance indicators in government hospital pharmacy departments in Ogun State.
