Effective safety management systems are critical to accident prevention in quarrying operations, yet compliance levels in Nigeria's small and medium-scale quarry sector remain poorly documented. This study analyses the adequacy and implementation of safety management systems at limestone quarry operations in Cross River State, South South Nigeria. A descriptive survey was conducted across eight quarry sites, employing safety audits, observation checklists, and structured interviews with 180 quarry workers and 25 site supervisors. Safety audit criteria were aligned with the International Labour Organization's Safety and Health in Mines Convention (C176) and Nigeria's Minerals and Mining Regulations 2011. Results indicate widespread deficiencies including the absence of formal emergency response plans, inadequate hazard identification processes, and limited worker participation in safety committees. Incident reporting mechanisms were non-functional in 75 percent of sites visited, creating information gaps that hinder continuous safety improvement. The study finds a significant correlation between the level of training investment and reduction in lost-time injury frequency rates. Recommendations include mandatory safety management system certification, government-facilitated training programmes for small-scale operators, and strengthened inspectorate capacity within the Federal Ministry of Mines and Steel Development. This study contributes essential field-level evidence to support safety governance reforms in Nigeria's quarrying sector.
