Background: Nurse attrition from rural health facilities in Nigeria's North West region threatens the delivery of primary healthcare services to underserved populations. Identifying retention barriers is critical for workforce planning. Aim: This study investigated factors affecting nurse retention in rural health facilities across selected local government areas in Kebbi State. Methods: A descriptive cross-sectional survey recruited 85 nurses from rural primary health care centres using purposive sampling. A self-administered questionnaire addressed professional motivators, working conditions, and personal push factors. Data were analysed using SPSS version 25 with logistic regression. Results: Poor remuneration, inadequate housing allowances, limited career development opportunities, and professional isolation were the most frequently cited retention barriers. Sixty-nine percent of respondents had considered relocating to urban centres or abroad. Significant associations were found between rural hardship allowances and intent to stay. Conclusion: Financial incentives and professional development opportunities are the most potent retention determinants. The study recommends a comprehensive rural nursing retention package including enhanced allowances, mentorship programmes, and telemedicine infrastructure for professional support.
