Foreign Aid, Conditionality, and Health System Strengthening in Anambra State: Navigating International Dependencies

Foreign aid conditionality, the practice of attaching policy reform requirements to international assistance, has been a persistent source of tension in donor-recipient relationships, raising fundamental questions about sovereignty, policy ownership, and the effectiveness of externally imposed conditions in achieving sustainable development outcomes. In the health sector, foreign aid conditionalities have shaped the design and implementation of primary healthcare, HIV treatment, reproductive health, and immunization programs across sub-Saharan Africa. Anambra State has received substantial health sector foreign assistance from bilateral donors and multilateral organizations, making it an informative context for examining conditionality dynamics at the subnational level. This study analyzes the influence of aid conditionality on health system strengthening initiatives in Anambra State between 2020 and 2024, drawing on interviews with state Ministry of Health officials, international donor program managers, and healthcare practitioners. The research is theoretically anchored in aid dependence theory and the Paris Declaration principles of aid effectiveness. Findings reveal that conditionalities attached to health aid in Anambra State have driven some positive governance improvements, particularly in financial management and reporting transparency, but have also generated distortions, including donor-priority diseases receiving disproportionate attention at the expense of broader primary healthcare needs. State health officials report limited negotiating power in aid design processes. The study recommends a partnership model that formally recognizes Anambra State's health system priorities as the primary reference point for aid program design, reducing conditionality-induced policy displacement.
