Health Systems Governance, Accountability, and Community Participation: A Comparative Analysis Across Nigerian States

Health systems governance and accountability mechanisms are fundamental determinants of health system performance, yet comparative analysis of governance quality and community participation across Nigerian states at different development trajectories remains a significant evidence gap. This study conducts a comparative health governance analysis across six purposively selected states representing varying levels of governance quality and health system performance: Lagos, Kano, Enugu, Rivers, Taraba, and Kebbi. A convergent parallel mixed-methods design is applied, combining quantitative governance quality assessments using the WHO Framework for Health Governance and Accountability applied to 90 facilities (15 per state), alongside qualitative comparative case studies in each state. Quantitative data collection includes structured audits of health facility governance documentation, budget transparency, accountability mechanisms, and community health committee functionality. Qualitative data are gathered from 180 key informant interviews with state health commissioners, facility managers, civil society representatives, and community health committee members. Data are analysed using cross-case comparison in NVivo for qualitative strands and multilevel regression in SPSS for quantitative comparisons. Health systems performance is correlated with governance scores using state-level health outcome indicators from the Nigeria State Health Investment for Equity health scorecard. The principal-agent theory and Democratic Accountability framework provide the theoretical basis. Original contributions include a comparative governance typology of Nigerian states and a replicable governance assessment toolkit.
