Political Leadership and Health Sector Reform in Anambra State

This study investigates the role of political leadership in driving health sector reforms in Anambra State, examining the policy decisions, institutional changes, and resource mobilisation strategies associated with health system improvements between 2014 and 2023. Anambra State has been cited in comparative state governance assessments as a relative high performer in health sector administration within the South East geopolitical zone, making it an instructive case for understanding how political commitment translates into health outcomes. Using a qualitative case study design, the study draws on official health policy documents, interviews with health administrators, public health officials, and community health workers across three health zones of the state. The leadership theory of public administration and the health governance framework are applied to examine the pathways through which political decisions affect service delivery. Findings indicate that sustained political commitment to primary health care financing, strategic deployment of diaspora expertise, and structured community health worker programmes contributed significantly to improvements in maternal and child health indicators in Anambra State. The study also documents governance challenges including corruption in procurement, inadequate health infrastructure in rural areas, and dependence on donor funding for specific programme areas. Recommendations include institutionalising multi-year health budgets, strengthening community health financing mechanisms, and establishing independent performance monitoring for primary health facilities.
