Local Government and Maternal Health Outcomes in Rural Communities: Analytical Evidence from Kogi and Kwara States

This study analytically examines the role of local government health expenditure, primary healthcare management, and community health program administration in determining maternal health outcomes in rural communities of Kogi and Kwara States, North Central Nigeria. Maternal mortality ratios in rural Nigeria remain among the highest globally, yet the specific governance mechanisms at the local government level that mediate the relationship between resource availability and health outcomes are empirically underexplored. Guided by the Health Systems Governance Framework and the Social Determinants of Health Model, this research collects primary data from 220 respondents across ten rural local government areas, including primary health care workers, community health extension workers, maternal health program beneficiaries, and local government health officers. Secondary data includes Kogi and Kwara State Primary Health Care Development Agency annual reports, Demographic and Health Survey Nigeria datasets, and academic literature on sub-national health governance in Nigeria. Structural equation modeling and regression analysis are employed. The study addresses a gap in the empirical literature on local governance determinants of maternal health at the rural local government level in North Central Nigeria. Findings are expected to demonstrate that local government health expenditure per capita, community health worker density, and antenatal care outreach frequency are the strongest governance predictors of maternal health outcomes. Recommendations include performance-based grants for local government health programs, community health worker pay reform, and integration of maternal health indicators into council performance evaluation.
